Dr. Bouwmeester is responsible for didactic and experiential teaching and maintains a clinic practice site at the Harbor Health Elder Service Plan which is a PACE (Program of All-inclusive Care for the Elderly) model of care. She manages clinical pharmacy services under a collaborative practice agreement within an interprofessional team setting. She also oversees medication utilization within the PACE program as a member of the quality assurance performance improvement (QAPI) committee. Additional clinical activities include immunization monitoring, comprehensive medication reviews, anticoagulation management, and medication education with an emphasis on dementia care. The interprofessional collaboration within the PACE organization and unique patient population (nursing home eligible older adults currently residing in a community setting) are ideal for implementing translational research. Her clinical practice and research interests focus on effective medication management and coordination of care for frail older adults residing in community settings. Dr. Bouwmeester is recognized as a fellow of the American Society of Consultant Pharmacists (ASCP) and served as chair of several national ASCP committees as well as president and board members of the Massachusetts chapter. She is also involved with the American Geriatrics Society and was recently co-chair of the Pharmacists Section.
