Overview

Dr. Carla Bouwmeester is a Clinical Professor in the Department of Pharmacy and Health System Sciences at Northeastern University with a clinical practice site at the Harbor Health Elder Service Plan which is a Program of All-inclusive Care for Elderly (PACE) organization in the Boston metro area. She works under a collaborative practice agreement to provide medication management services and quality improvement programs related to fall prevention, transitions of care, medication optimization, chronic disease management, and behavioral symptoms related to dementia.
